WHEREAS, the Clerks of Court are responsible for filing information returns (Form 1099) with the Internal Revenue Service for interest collected and paid on Judgments, and
WHEREAS, an appropriate procedure is necessary to acquire requisite information for purposes of completing information returns (Form 1099),
NOW THEREFORE, I, RAUL A. ZAMBRANO, Chief Judge of the Seventh Judicial Circuit, hereby order that parties/ litigants applying for payment of monies from Judgments are to provide the Clerk of Court with a valid Social Security or Tax ID number.
TO BE RECORDED in Flagler, Putnam, St. Johns and Volusia counties.
