IN THE CIRCUIT COURT OF FLORIDA, SEVENTH JUDICIAL CIRCUIT IN AND FOR FLAGLER, PUTNAM, ST. JOHNS AND VOLUSIA COUNTIES

Re: SUPPORT INFORMATION SHEETS (Amended)

Ref: FM-2006-024-SC (A) (Rescinds FM-2006-024-SC)

WHEREAS, pursuant to Chapter 61, Florida Statutes, certain pertinent information is required to be included in all judgments related to child support and/or alimony, and

WHEREAS, the inclusion of this information is necessary for the efficient operation of the respective Clerks of Court offices in the Seventh Judicial Circuit;

NOW THEREFORE, I, WILLIAM A. PARSONS, Chief Judge of the Seventh Judicial Circuit, hereby order that a completed Support Information Sheet be filed with and made part of every Final Judgment, Modification of Final Judgment, and/or Post Judgment Decree regarding child support and/or alimony issued in the Seventh Judicial Circuit.  The Support Information Sheet must be in a format substantially similar to that attached hereto as Attachment A (PDF).

IT IS FURTHER ORDERED that Administrative Order # FM-2006-024-SC is hereby rescinded.

TO BE RECORDED in Flagler, Putnam, St. Johns and Volusia counties.

DONE AND ORDERED in Daytona Beach, Volusia County, Florida this 21st day of April 2006.
