IN THE CIRCUIT COURT OF FLORIDA, EVENTH JUDICIAL CIRCUIT IN AND FOR FLAGLER, PUTNAM, ST. JOHNS AND VOLUSIA COUNTIES

Re: APPROVAL OF LOCAL FAMILY LAW FORMS

Ref: FM-2002-010-SC

WHEREAS, the Supreme Court of Florida adopted Rule 12.750, Florida Family Law Rules of Procedure, which authorizes chief judges to approve in writing forms in substantial compliance with and not inconsistent with the Supreme Court approved forms, and

WHEREAS, such forms, which include local requirements, have been drafted and reviewed by the Family Law judges, and

WHEREAS, the undersigned has been advised of the necessity to approve such forms for use throughout the circuit;

NOW, THEREFORE, I, ROBERT K. ROUSE JR., Chief Judge of the Seventh Judicial Circuit of Florida, hereby approve the forms listed in Attachment A, attached hereto, for use within the Seventh Judicial Circuit.

IT IS FURTHER ORDERED that forms may be added to, or deleted from, the list of approved forms, upon approval of the Family Law judges, without benefit of a revised Administrative Order.  In the event the list of approved forms is amended, the Court Administrator’s office shall update said list and circulate it to interested parties, including those designated by Rule 12.750, Florida Family Law Rules of Procedure.
